Tas neatbilst labas p\u0101rvald\u012bbas un l\u012bdzdal\u012bbas principiem, kas paredz profesion\u0101lo organiz\u0101ciju iesaisti jau politikas izstr\u0101des s\u0101kumposm\u0101, nevis tikai publisk\u0101s apsprie\u0161anas f\u0101z\u0113.<\/li>\n\n\n\n<li><strong>Optometrista k\u0101 funkcion\u0101l\u0101 speci\u0101lista juridiskais statuss ir ignor\u0113ts<\/strong>. Saska\u0146\u0101 ar \u0100rstniec\u012bbas likuma 45.\u00b9 pantu optometrists ir \u0101rstniec\u012bbas persona \u2013 funkcion\u0101lais speci\u0101lists. Likuma 45.\u00b2 panta pirm\u0101s da\u013cas 2. punkts nosaka, ka funkcion\u0101lais speci\u0101lists veic \u0101rst\u0113\u0161anu, izmantojot atbilsto\u0161u diagnostiku un nov\u0113rt\u0113\u0161anu, un sniedz atzinumus savas kompetences ietvaros. Tom\u0113r vair\u0101ki sp\u0113k\u0101 eso\u0161ie Ministru kabineta noteikumi joproj\u0101m ierobe\u017eo optometrista kompeten\u010du izmanto\u0161anu, un \u0161\u012b neatbilst\u012bba ir p\u0101rnesta ar\u012b SAVA nolikuma projekt\u0101.<\/li>\n\n\n\n<li><strong>SAVA nolikums neatbilst starptautiskaj\u0101m (PVO) redzes apr\u016bpes sist\u0113mu rekomend\u0101cij\u0101m<\/strong>. Jau laika period\u0101 no 2019. l\u012bdz 2022. gadam Pasaules vesel\u012bbas organiz\u0101cija (PVO) ir public\u0113jusi vair\u0101kus dokumentus, kas nosaka m\u016bsdien\u012bgas redzes apr\u016bpes sist\u0113mas princius (piem\u0113ram, <em>Integrated People-Centred Eye Care<\/em> (IPEC), <em>Eye Care Competency Framework<\/em> (ECCF), rekomend\u0101cijas par uzdevumu p\u0101rdali (<em>task shifting<\/em>) un darbasp\u0113ka efekt\u012bvu izmanto\u0161anu). \u0160ie dokumenti kopum\u0101 veido visaptvero\u0161u metodolo\u0123isko b\u0101zi modernas redzes apr\u016bpes sist\u0113mas izveidei, nodro\u0161inot saikni starp prim\u0101ro, sekund\u0101ro un terci\u0101ro apr\u016bpes l\u012bmeni (Pielikums Nr. 1). NVD virz\u012btais SAVA nolikums ir izstr\u0101d\u0101ts, ne\u0146emot v\u0113ra \u0161\u012bs rekomend\u0101cijas un balst\u012bts tikai uz oftalmologa kompetenci gan prim\u0101raj\u0101, gan sekund\u0101raj\u0101, gan terci\u0101raj\u0101 apr\u016bp\u0113. Latvijas pa\u0161reiz\u0113jais un SAVA nolikum\u0101 pl\u0101notais redzes apr\u016bpes modelis \u0161\u012bm vadl\u012bnij\u0101m neatbilst.<\/li>\n\n\n\n<li><strong>SAVA nolikums paredz nesam\u0113r\u012bgu un ekonomiski neefekt\u012bvu valsts bud\u017eeta l\u012bdzek\u013cu izlieto\u0161anu<\/strong>. Visu valsts apmaks\u0101to redzes apr\u016bpes pakalpojumu koncentr\u0113\u0161ana augstas kvalifik\u0101cijas speci\u0101listu (oftalmologu) rok\u0101s padara valsts nodro\u0161in\u0101to redzes apr\u016bpes pakalpojumu nesam\u0113r\u012bgi d\u0101rgu un samazina t\u0101 pieejam\u012bbu (nodro\u0161inot maz\u0101ku valsts nodro\u0161in\u0101to redzes apr\u016bpes viz\u012b\u0161u skaitu).<\/li>\n<\/ol>\n\n\n\n<p>Kl\u0101t pievienojam (Pielikum\u0101&nbsp; Nr. 2) LOOA 2025. gada 22. decembr\u012b nos\u016bt\u012bto v\u0113stuli NVD, kur\u0101 detaliz\u0113ti izkl\u0101st\u012bti juridiski, profesion\u0101li un ekonomiski argumenti par konstat\u0113tajiem tr\u016bkumiem NVD r\u012bc\u012bb\u0101.<\/p>\n\n\n\n<p>T\u0101pat atg\u0101din\u0101m, ka, neskatoties uz m\u016bsu vair\u0101kk\u0101rt\u0113jiem priek\u0161likumiem, joproj\u0101m nav nov\u0113rsta neatbilst\u012bba starp \u0100rstniec\u012bbas likumu, kas nosaka optometristu k\u0101 neatkar\u012bgu funkcion\u0101lo specialistu, un vair\u0101kiem Ministru kabineta noteikumiem, kas diskrimin\u0113 optometristu k\u0101 funkcion\u0101lo speci\u0101listu un liedz tam sniegt pakalpojumus, kas ir t\u0101 kompetenc\u0113 un atbilst noteikumos pras\u012btaj\u0101m prasm\u0113m. \u0160obr\u012bd virkn\u0113 Ministru kabineta noteikumu, kas regul\u0113 iedz\u012bvot\u0101ju oblig\u0101t\u0101s un profilaktisk\u0101s vesel\u012bbas p\u0101rbaudes, redzes funkciju nov\u0113rt\u0113\u0161ana ir uztic\u0113ta vien\u012bgi oftalmologiem. Tas rada nevienl\u012bdz\u012bgu situ\u0101ciju \u0101rstniec\u012bbas personu vid\u016b un nelietder\u012bgi noslogo oftalmologus ar funkcion\u0101lu redzes p\u0101rbau\u017eu veik\u0161anu, ko pilnv\u0113rt\u012bgi var nodro\u0161in\u0101t optometristi. LOOA aicina Vesel\u012bbas ministriju rosin\u0101t groz\u012bjumus \u0161\u0101dos normat\u012bvajos aktos:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Ministru kabineta<\/strong> 2018. gada 28. augusta <strong>noteikumos Nr. 555<\/strong> Vesel\u012bbas pakalpojumu organiz\u0113\u0161ana un samaksa iek\u013caut optometristu speci\u0101listu sarakst\u0101, kas ir ties\u012bgi veikt valsts apmaks\u0101tu redzes skr\u012bningu un profilaktisk\u0101s apskates b\u0113rniem un pieaugu\u0161ajiem. Paredz\u0113t optometristu k\u0101 speci\u0101listu, pie kura \u0123imenes \u0101rsts var nos\u016bt\u012bt pacientu funkcion\u0101lai redzes izv\u0113rt\u0113\u0161anai sekund\u0101r\u0101s ambulator\u0101s apr\u016bpes ietvaros. Uz \u0161o nepiecie\u0161am\u012bbu nor\u0101d\u012bjis NVD v\u0113stul\u0113 Nr. 16-7\/12309\/2020, ko jau 2020.\u00a0gad\u0101 sa\u0146\u0113ma LOOA. Cit\u0113jot v\u0113stules tekstu: \u201c<em>Dienests paskaidro, ka, lai optometristi saska\u0146\u0101 ar normat\u012bvajiem aktiem var\u0113tu veikt profilaktisk\u0101s apskates (valsts apmaks\u0101tu pakalpojumu ietvaros), ir nepiecie\u0161ams veikt groz\u012bjumus Noteikumos Nr.555, lai optometristu sniegtie pakalpojumi tiktu iek\u013cauti valsts apmaks\u0101tas vesel\u012bbas apr\u016bpes pakalpojumu kl\u0101st\u0101.\u201d<\/em><\/li>\n\n\n\n<li><strong>Ministru kabineta<\/strong> 2009. gada 10. marta <strong>noteikumu Nr. 219<\/strong> Oblig\u0101to vesel\u012bbas p\u0101rbau\u017eu (OVP) noteikumos papla\u0161in\u0101t optometrista lomu ne tikai piez\u012bm\u0113s par darbu ar datoru (1. pielikuma 4.10. un 4.11. punkti), bet ar\u012b pamattekst\u0101 k\u0101 speci\u0101listu, kur\u0161 ties\u012bgs veikt redzes p\u0101rbaudes darb\u0101 augstum\u0101 (2. pielikums) un citos gad\u012bjumos ar palielin\u0101tu redzes slodzi. Jau 2020.\u00a0gad\u0101 NVD v\u0113stul\u0113 Nr. 16-7\/12309\/2020 inform\u0113ja LOOA, ka <em>\u201catbilsto\u0161i normat\u012bvajos aktos noteiktajai optometrista visp\u0101r\u0113jai kompetencei, optometristiem ir ties\u012bbas veikt profilaktisk\u0101s apskates.\u201d<\/em><\/li>\n\n\n\n<li><strong>Ministru kabineta<\/strong> 2011. gada 6. decembra <strong>noteikumu Nr. 940<\/strong> Transportl\u012bdzek\u013cu vad\u012bt\u0101ju vesel\u012bbas p\u0101rbau\u017eu noteikumos (7. punkts) iek\u013caut optometristu \u0101rstu komisijas sast\u0101v\u0101, \u013caujot tam veikt funkcion\u0101lo redzes nov\u0113rt\u0113\u0161anu (redzes asums, redzes lauks) pirmreiz\u0113j\u0101m un k\u0101rt\u0113j\u0101m p\u0101rbaud\u0113m.<\/li>\n\n\n\n<li><strong>Ministru kabineta<\/strong> 2024. gada 24. septembra <strong>noteikumu Nr. 622<\/strong> Iesaucamo vesel\u012bbas st\u0101vok\u013ca pras\u012bbu un vesel\u012bbas p\u0101rbau\u017eu sarakst\u0101 (1. pielikums) ietvert optometristu k\u0101 speci\u0101listu, kur\u0161 ir ties\u012bgs veikt prim\u0101ro redzes asuma un refrakcijas izmekl\u0113\u0161anu.<\/li>\n\n\n\n<li><strong>Ministru kabineta<\/strong> 2023. gada 25. apr\u012b\u013ca <strong>noteikumu Nr. 202<\/strong> Iero\u010du glab\u0101t\u0101ju un apsardzes sertifik\u0101tu pretendentu vesel\u012bbas p\u0101rbaud\u0113s (3.2. apak\u0161punkts) paredz\u0113t optometrista dal\u012bbu komisij\u0101 redzes funkcion\u0101l\u0101 st\u0101vok\u013ca izv\u0113rt\u0113\u0161anai.<\/li>\n<\/ol>\n\n\n\n<p>LOOA priek\u0161likumi prec\u012bziem MK noteikumu groz\u012bjumiem skat\u0101mi pielikum\u0101 nr. 3.<\/p>\n\n\n\n<p>LOOA uzskata, ka \u0161\u0101di normat\u012bvo aktu groz\u012bjumi veicin\u0101s redzes apr\u016bpes sist\u0113mas moderniz\u0101ciju Latvij\u0101, lai padar\u012btu redzes apr\u016bpi pieejam\u0101ku, kvalitat\u012bv\u0101ku, finansi\u0101li efekt\u012bv\u0101ku un atbilsto\u0161\u0101ku valsts vesel\u012bbas politikai, kas ir tie\u0161s Vesel\u012bbas ministrijas pien\u0101kums:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>likvid\u0113s redzes apr\u016bp\u0113 iesaist\u012bto profesiju nevienl\u012bdz\u012bbu redzes apr\u016bpes sist\u0113m\u0101, iev\u0113rojot katras profesijas kompetences;<\/li>\n\n\n\n<li>samazin\u0101s rindas uz valsts apmaks\u0101tiem oftalmologu pakalpojumiem, k\u0101 rezult\u0101t\u0101 pieejam\u0101ka k\u013c\u016bs specializ\u0113t\u0101 pal\u012bdz\u012bba pacientiem ar acu slim\u012bb\u0101m. Optometrists savuk\u0101rt nodro\u0161in\u0101s prec\u012bz\u0101ku un savlaic\u012bg\u0101ku redzes funkciju (refrakcijas, binokul\u0101ro un akomod\u0101cijas funkciju u.c.) nov\u0113rt\u0113\u0161anu un korekciju savu kompeten\u010du ietvaros. Pie tam optometrista zin\u0101\u0161anas un prasmes ir pietiekamas ar\u012b acs vesel\u012bbas st\u0101vok\u013ca skr\u012bningam, lai atlas\u012btu pacientu grupu, kuriem ir nepiecie\u0161ama oftalmologa konsult\u0101cija. Tas \u013caus efekt\u012bv\u0101k organiz\u0113t ne tikai redzes apr\u016bpes procesu, bet ar\u012b \u0101rstniec\u012bbas personu resursu izmanto\u0161ana atbilsto\u0161i \u0100rstniec\u012bbas likumam.<\/li>\n<\/ol>\n\n\n\n<p>Jau kop\u0161 Latvijas neatkar\u012bbas atjauno\u0161anas laikiem redzes apr\u016bpes sist\u0113m\u0101 oftalmologs ir vien\u012bgais valsts apmaks\u0101tais tie\u0161\u0101s pieejam\u012bbas speci\u0101lists. Jaunais SAVA nolikums ar\u012b turpm\u0101k paredz gad\u0101 apmaks\u0101t 220 941 ambulator\u0101s viz\u012btes tikai pie oftalmologa (aptuveni 12 000 viz\u012btes gad\u0101 uz 100 000 iedz\u012bvot\u0101jiem). T\u0101l\u0101ka \u0161\u0101da uz vienu redzes apr\u016bpes speci\u0101listu v\u0113rsta mode\u013ca uztur\u0113\u0161ana neatbilst m\u016bsdien\u012bgas vesel\u012bbas apr\u016bpes sist\u0113mas pras\u012bb\u0101m un tas atbilsto\u0161i PVO p\u0113t\u012bjumiem un vadl\u012bnij\u0101m rada b\u016btiskus sist\u0113miskus riskus, kas negat\u012bvi ietekm\u0113s acu slim\u012bbu \u0101rstniecisko pieejam\u012bbu:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Ja uz 100 000 iedz\u012bvot\u0101jiem tiek nodro\u0161in\u0101tas tikai aptuveni 12 000 valsts apmaks\u0101tas viz\u012btes, tas <strong>sedz tikai nelielu da\u013cu no re\u0101l\u0101s vajadz\u012bbas<\/strong>. Tas rada p\u0101rkl\u0101juma plaisu: PVO dati liecina, ka tikai presbiopijas (vecuma redzes) un b\u0113rnu skr\u012bninga nodro\u0161in\u0101\u0161anai vien nepiecie\u0161ams iev\u0113rojami liel\u0101ks viz\u012b\u0161u skaits. K\u0101 rezult\u0101t\u0101 iedz\u012bvot\u0101ji ar zem\u0101kiem ien\u0101kumiem, lauku re\u0123ionu iedz\u012bvot\u0101ji un seniori paliek bez apr\u016bpes, kas palielina &#8220;neapmierin\u0101to vajadz\u012bbu&#8221; un nevienl\u012bdz\u012bbas apjomu.<\/li>\n\n\n\n<li>Eso\u0161\u0101 redzes apr\u016bpes mode\u013ca t\u0101l\u0101ka uztur\u0113\u0161ana <strong>nov\u0101jina prim\u0101ro redzes apr\u016bpi un nenodro\u0161ina pietiekamu acu slim\u012bbu skr\u012bningu<\/strong>. PVO strat\u0113\u0123ija &#8220;<em>Integrated people-centered eye care<\/em>&#8221; (IPEC) uzsver nepiecie\u0161am\u012bbu veicin\u0101t uzdevumu p\u0101rvirz\u012b\u0161anu speci\u0101li apm\u0101c\u012btam person\u0101lam, lai uzlabotu apr\u016bpes pieejam\u012bbu un atdal\u012btu prim\u0101ro un sekund\u0101ro vesel\u012bbas apr\u016bpes posmus. Anal\u012bze r\u0101da, ka oftalmologa apr\u016bpes epizode pa\u0161laik oblig\u0101ti ietver refrakcijas noteik\u0161anu, redzes funkciju nov\u0113rt\u0113\u0161anu un redzes korekciju, kas tie\u0161i atbilst optometrista kompetenc\u0113m. Vid\u0113ji \u0161\u012bs darb\u012bbas aiz\u0146em 15\u201320 min\u016btes no viz\u012btes, kas noz\u012bm\u0113, ka oftalmologs zaud\u0113 30\u201350\u00a0% sava laika, veicot optometrista darbu.<\/li>\n\n\n\n<li>Tikai oftalmologu iesaiste valsts apmaks\u0101ta redzes apr\u016bpes pakalpojuma nodro\u0161in\u0101\u0161an\u0101 <strong>rada augstas kvalifik\u0101cijas speci\u0101listu noslodzi ar neadekv\u0101tiem uzdevumiem<\/strong>. Tie\u0161\u0101 pieejam\u012bba oftalmologam bez optometrista k\u0101 prim\u0101r\u0101 filtra rada &#8220;pudeles kakla&#8221; efektu. Oftalmologi (\u0137irurgi un specializ\u0113tie \u0101rsti) tiek noslogoti ar refrakcijas k\u013c\u016bdu kori\u0123\u0113\u0161anu un vienk\u0101r\u0161\u0101m s\u016bdz\u012bb\u0101m (piem\u0113ram, astenopisk\u0101s s\u016bdz\u012bbas vai saus\u0101s acs s\u016bdz\u012bbu risin\u0101\u0161ana), kas PVO ECCF (<em>eye care competency framework<\/em>) ietvar\u0101 ir optometrista autonom\u0101 kompetence. Tas tie\u0161i pagarina gaid\u012b\u0161anas laiku uz kritiski svar\u012bg\u0101m intervenc\u0113m, piem\u0113ram, diab\u0113tisk\u0101s retinop\u0101tijas, glaukomas vai citu acu slim\u012bbu \u0101rst\u0113\u0161anu, kur savlaic\u012bga diagnostika un \u0101rst\u0113\u0161anas uzs\u0101k\u0161ana ir iz\u0161\u0137iro\u0161s faktors redzes saglab\u0101\u0161anai. T\u0101pat rindu d\u0113\u013c oftalmologs nav pieejams ak\u016btu un neatliekamu st\u0101vok\u013cu gad\u012bjumos, piem\u0113ram, sve\u0161\u0137ermenis ac\u012b vai keratokonjunktiv\u012bts.
